Hospice 101

Hospice’s goal is to administer care, comfort, and dignity to patients approaching the end of their lives. Unlike other modes of care, hospice is not designed to cure the patient’s illness, but maximize the quality of life in their final years.

Hospice is available to patients with a life-limiting illness and a life expectancy of six months or less, who have decided to stop curative treatments of their condition, or who have experienced significant cognitive or physical decline.

Hospice services include pain and symptom management, respite care, home health aides, counseling, and bereavement support. Care is administered by a multi-disciplinary team that assists with physical, familial, and spiritual matters.

Usually, Medicare pays for hospice care through the Medicare Hospice Benefit. Once this benefit starts, Original Medicare will cover hospice services, even if you’re enrolled in a Medicare Advantage Plan or other Medicare health plan. VA benefits, Medi-Cal (California’s Medicaid program), and private insurance also cover hospice care.

Hospice providers must be licensed by the California Department of Public Health (CDPH) and certified by the Centers for Medicare and Medicaid Services (CMS). They must also be accredited by State agencies or an Accrediting Organization (AO).

In the Arms of Grace Hospice holds these licenses and certifications, and is accredited by The Joint Commission (TJC), the nation’s leading Accrediting Organization that continually surveys and evaluates healthcare organizations to determine compliance with national standards of safety and quality of care. We are also a member of the California Hospice and Palliative Care Organization and the National Alliance for Care at Home.

Family education should take place on an ongoing basis, with every visit. It shouldn’t amount to a one-time crash course on hospice care.

At In the Arms of Grace Hospice, we believe in the constant education and re-education of our families. We train our patients’ families to properly provide care, administer medication, and look out for signs of disease progression. Our team works hands-on with families to make sure they’re comfortable with caregiver duties.

If the patient decides to designate their primary physician as their attending physician while enrolled in hospice, that physician will act as a part of the patient’s hospice team. They will be kept in the loop with the patient’s plan and be consulted regarding the patient’s care plan.

If the patient lives with the family, the family will have a first-hand view of the patient’s condition. If the loved one lives and receives care in a facility, the patient’s care coordinator will keep the family up to date on the patient’s condition. If the patient’s conditions change, any interventions will be discussed with the family.

At In the Arms of Grace Hospice, we pride ourselves on this communication. We keep our patient-staff ratio low, so our teams have the bandwidth to make sure everyone is up to speed and in-the-know about the patient’s care plan.

Hospice care teams are on-call 24/7 for their patients to respond to any emergency. One advantage of a holistic hospice care team is the pre-planning involved around the types of emergencies that are most likely to happen. Our team works to identify issues when they are small and mitigate them so an emergency is less likely to occur in the first place.

The hospice provider ideally maintains a low patient-staff ratio, so hospice teams have time to give each patient and their families the time they deserve. Education and communication should be thorough and frequent. Staff and volunteers should be kept on the cutting edge of best industry practices through ongoing orientation and training.

Hospice began as a grassroots, volunteer practice. Today volunteer hours must equal 5% of total patient care hours provided by staff. These volunteers can support patients by reading to them, taking walks with them, and other activities. They may assist families by helping with shopping, pet care, light housework, and other tasks that give family caregivers some relief.

Our volunteers undergo extensive orientation and training to understand the hospice philosophy and mission, consistent with industry standards, and to meet the needs of patients and families.
